- You need a psychiatrist (MD/DO) direct evaluation right now: at launch, shrinkMD's practicing clinician is a PMHNP under psychiatrist oversight. For MD/DO-direct evaluation in Maine or Nebraska, ask your primary-care physician for a referral, or use the American Psychiatric Association's find-a-psychiatrist directory. Wait times are typically 60 to 120 days.
- You need controlled substances: shrinkMD does not prescribe stimulants or benzodiazepines. Any local psychiatrist accepting new patients can, and community mental-health centers routinely handle these.
- You need in-person care, hospitalization, or IOP: in Maine, Spring Harbor Hospital (Greater Portland) and Northern Light Acadia (Bangor); in Nebraska, Nebraska Medicine, CHI Health Immanuel, and Bryan West Behavioral Health.
- You are under 18: shrinkMD is adults-only. Boys Town National Research Hospital in Nebraska serves adolescents; Spring Harbor Hospital in Maine has adolescent services.

Does shrinkMD have a psychiatrist (MD or DO) taking new patients?
At launch, shrinkMD's practicing clinician is a board-certified PMHNP (Matthew George) under psychiatrist medical direction (Dr. Shariq Refai, Medical Director). Direct psychiatrist appointments will open as our clinician panel expands. If you specifically need to see an MD or DO psychiatrist right now, our "If shrinkMD is not the right fit yet" section above lists honest alternatives.
What's the difference between a psychiatrist and a PMHNP?
Both are trained to evaluate, diagnose, and prescribe medication for psychiatric conditions. A psychiatrist is a physician (MD or DO) with residency training in psychiatry. A PMHNP is a psychiatric mental health nurse practitioner with graduate-level psychiatric training. In Nebraska and Maine, PMHNPs practice with full or collaborative authority depending on the setting. Clinical outcomes for outpatient medication management are broadly comparable when the clinician is board-certified and experienced.
What conditions does shrinkMD treat?
Adult depression, anxiety, bipolar spectrum disorders, OCD, PMDD, postpartum depression, sleep problems, adjustment disorder, and related conditions. We do not treat under 18, we do not prescribe controlled substances (stimulants for ADHD, benzodiazepines), and we do not do in-person care.
What states does shrinkMD currently serve?
Maine and Nebraska at launch. Florida, Georgia, Texas, California, New York, Virginia, Indiana, and Hawaii are in various stages of clinician licensure and will open as licensure completes. See By state above for current status.
