How to use it

Five minutes now, thirty minutes saved later

Fill in what you can remember. Add current medications one row at a time. Add past medications the same way (name, roughly what dose, roughly how long you took it, and roughly why you stopped). Leave anything blank you do not remember. Bad guesses are more useful than nothing.

When you are done, click "Build my summary." You will see a clean version of what you entered. Print it, save it as a PDF (your browser's print dialog has a "Save as PDF" destination), email it to yourself, or copy the text to clipboard and paste it wherever you want to keep it. Bring the printed copy or your phone to your first visit.

Is anything sent to shrinkMD when I fill this out?

No. Everything stays in your browser unless you specifically choose to email yourself the summary at the end.

Do I have to be a shrinkMD patient to use this?

No. It is designed to be useful for any first psychiatric visit, whether you are seeing shrinkMD, another practice, a therapist, or your PCP.

What if I do not remember some of my past medications?

Estimate. "An SSRI, I think it was Zoloft, I took it in 2018 or 2019 for about six months and stopped because of side effects" is more useful than nothing.

Do I include supplements and over-the-counter?

Yes if you take them specifically for mental health (melatonin, St. John's Wort, ashwagandha, magnesium for sleep, etc.). Otherwise your clinician will ask about the rest separately.

Can I print this?

Yes. Click "Print or save as PDF." The site's nav and footer are hidden from the print output so you get a clean one-pager.
