Nebraska Medicaid (Heritage Health) covers psychiatric care and expanded telehealth coverage during and after the pandemic. Commercial coverage in the state is dominated by Blue Cross and Blue Shield of Nebraska, UnitedHealthcare, and Aetna, with mental health parity required under federal MHPAEA. Out-of-network mental-health benefits vary substantially across plans; some plans have essentially no out-of-network benefit for outpatient mental health, others cover 50 to 80 percent after deductible. Calling the number on your member ID card and asking two specific questions (percentage covered out of network, and current out-of-network deductible) answers whether cash-pay plus superbill is realistic for you.
