What we don’t treat:

Patients who are emergently seeking care (recently hospitalized, suicidal, under the influence of a substance due to struggle with substance abuse, etc.), patients who self-harm, eating disorders, and personality disorders. These should be treated by someone who can provide in person care locally.

Our Service Fees:

All fees are due at the time of service.

All fees are due at the time of service. A deposit of $150 is required for new patients in order to hold the appointment time. The deposit will be allocated to the total fee at the time of service. 48 hours is required to cancel a new patient appointment or to reschedule otherwise deposit is forfeited. A new patient appointment will be cancelled if deposit is not paid within 3 business days of scheduling appointment.

Your account balance must be settled by the end of each visit in order to schedule a follow up appointment. If you have an outstanding balance with this office, you will not be allowed to schedule your follow up until your balance has been paid in full.

We do not participate in health insurance plans. This is of benefit to our patients. This provides for more confidentiality as information does not have to be shared with insurance companies or people who don’t need to know about the care you receive. Sensitive patient information can be kept confidential. It also allows for comprehensive personalized care. We are considered an out of network provider when it comes to insurance. This is to ensure we can provide the highest quality of care for you without insurance dictating your care. Due to this we expect payment for your appointment up front. If you do have health insurance, you may be able to get reimbursed from your health insurance company. You would submit a form to your insurance company which you would obtain from your insurance company along with an itemized receipt we provide to you and send it to your insurance company. After this, your insurance company should reimburse you for your appointment. We value keeping your records completely confidential, however if you are submitting a claim to your insurance company, we are then required to inform them of your diagnosis and type of treatment that we are providing.

Most HSA (health savings account) and FSA (flexible spending account) reimburses for your office visit. However, best to check with your employer’s plan before assuming so.

How do I get reimbursed?

First call your insurance company to get details on your plan. Specifically ask what kind of coverage they provide for out of network providers. Find out what deductible you might have (if you have a deductible plan and the deductible is high the cost would be the same whether in network or out of network if you have no other other medical expenses to reach the deductible amount). You should also ask specifics on the process of getting reimbursed. We will provide a superbill for you to use for the reimbursement.
